Treatment Summaries & Care Plans

 

It is important for all cancer patients to have a treatment summary and care plan.

 

The treatment summary is a record of your cancer treatment.  It includes a detailed listing of your diagnosis and staging, any surgery, chemotherapy including the drug name and dosage, and any radiation or hormonal therapy. 

 

The care plan is an essential component of healthy survivorship.  It should include follow-up appointments with your oncologist, future surveillance screenings, any potential late or long-term effects of treatment, recommendations for exercise and nutrition, and a list of support resources.

 

The treatment summary and care plan should be given to your primary care physician and any other doctors you have.
